breast reduction surgery

Breast reduction surgery can make a significant difference to both physical comfort and confidence. However, it is important to understand that the result develops gradually. Immediately after surgery, the breasts will be swollen and the scars will be new, so they will not yet represent the final appearance.

Before proceeding, your surgeon should explain what can realistically be achieved, where the scars will be positioned and what to expect during recovery. The operation is tailored to the individual, taking into account the size and shape of the breasts, the amount of tissue being removed and the result the patient hopes to achieve.

What Does Breast Reduction Surgery Involve?

Breast reduction surgery, also known as reduction mammoplasty, removes excess breast tissue, fat and skin. The remaining breast tissue is then reshaped and the nipples are repositioned to create breasts that are smaller, lighter and better proportioned to the patient’s body.

The operation can help people who experience problems associated with large or heavy breasts, including:

  • Back, neck or shoulder discomfort
  • Bra straps digging into the shoulders
  • Skin irritation beneath the breasts
  • Difficulty exercising comfortably
  • Problems finding suitably fitting clothing
  • Self-consciousness about breast size or proportion

A breast reduction also incorporates an uplift because the remaining breast tissue and skin must be reshaped. However, unlike a breast uplift performed on its own, a reduction removes a more substantial amount of breast tissue.

What Do Breasts Look Like Immediately After a Reduction?

The breasts usually appear higher, firmer and smaller immediately after surgery, but they will also be swollen. They may initially look quite rounded or sit higher on the chest than expected.

This early appearance is not the final result. As swelling reduces and the tissues settle, the breasts normally soften and assume a more natural position.

Some difference between the two breasts may also be more noticeable during the early stages of recovery. No pair of breasts is perfectly identical before surgery, and absolute symmetry cannot be guaranteed afterwards. The aim is to produce a balanced and proportionate result while recognising the natural differences between each side.

When Will I See My Final Breast Reduction Results?

The improvement in breast size is visible immediately, but the final shape takes longer to develop.

During the first few weeks, swelling, bruising and tightness can affect the appearance of the breasts. Over the following months, the swelling gradually subsides and the breast tissue settles.

Most patients will have a good indication of their result after several months, although subtle changes can continue for longer. Scars also take considerably longer to mature than the breast shape itself.

Results vary according to the amount of tissue removed, skin quality, healing and the patient’s original anatomy. Your surgeon should show you relevant before-and-after photographs during your consultation and explain what may be achievable in your particular case.

Where Are Breast Reduction Scars?

All breast reduction surgery leaves permanent scars. Their position and extent depend on the technique used and the amount of skin and breast tissue that needs to be removed.

Common scar patterns include:

A scar around the areola

This follows the border between the darker areolar skin and the surrounding breast.

A vertical scar

This extends from the bottom of the areola to the crease beneath the breast. Together with the scar around the areola, this is sometimes described as a “lollipop” scar.

An anchor-shaped scar

For a larger reduction, an additional horizontal scar may be required along the breast crease. This creates an anchor-shaped pattern around the areola, vertically down the breast and across the crease.

The technique is selected according to the individual breast shape and the degree of reduction required. A shorter scar is not automatically the best choice if it would compromise the shape or result.

BAAPS advises that all breast reduction techniques leave scars, with larger reductions commonly requiring an anchor-shaped incision.

How Do Breast Reduction Scars Change?

Breast reduction scars are usually red or pink and may feel firm, raised or slightly lumpy during the early healing period. They should gradually become softer, flatter and paler.

Scar maturation is slow and can take 12 to 18 months. Some scars remain more visible than others, and individual healing cannot be predicted precisely.

Factors that can affect scarring include:

  • Genetics and skin type
  • Smoking
  • Infection or delayed wound healing
  • Tension on the healing incision
  • Sun exposure
  • A personal tendency to develop raised or widened scars

You will be given specific instructions about dressings and scar care. Products or massage should only be introduced once the wounds are sufficiently healed and your surgical team has confirmed that it is safe to do so.

Breast Reduction Recovery: What to Expect

Recovery is different for every patient, but it is sensible to arrange help at home and allow time away from work.

The first few days

The breasts are likely to feel sore, swollen and tight. Dressings will be applied and you may be asked to wear a supportive post-surgical bra.

Gentle movement is encouraged, but you should avoid reaching, stretching or lifting. Pain relief should be taken as advised.

The first two weeks

Bruising and swelling should begin to improve, although the breasts may still feel tender. Many patients need around two to three weeks away from work, depending on the nature of their job.

You will attend follow-up appointments so that the wounds and healing can be checked.

Weeks three to six

Everyday activities can gradually be increased, following your surgeon’s advice. Heavy lifting, strenuous exercise and upper-body activity must still be avoided until you have been cleared to resume them.

The NHS advises that complete initial recovery can take approximately two to six weeks and that strenuous exercise, stretching and heavy lifting may need to be avoided for up to six weeks.

After six weeks

Many patients can return to most normal activities around this stage, provided healing has progressed well. Some residual swelling may remain, and the breasts will continue to soften and settle over the coming months.

These timings are only a general guide. Your recovery plan will depend on the operation performed and how your body heals.

Will Breast Reduction Results Last?

Breast reduction can produce a long-lasting improvement in breast size, shape and comfort. However, the breasts will continue to change naturally over time.

Ageing, gravity, pregnancy, breastfeeding and significant weight fluctuations can all affect the result. Maintaining a stable weight can help preserve the new breast shape.

It is also important to continue normal breast awareness and attend routine screening when invited. Surgery does not prevent future breast changes or breast disease.

Can I Choose My New Breast Size?

You can discuss the size and proportion you would prefer, but it is not possible to guarantee a particular bra cup size. Cup sizing varies between manufacturers and does not provide an exact surgical measurement.

The priority is to create a result that is proportionate, safe and suitable for your body. The amount that can be removed may also be influenced by the blood supply to the nipple and the quality of the remaining tissue.

Mr Wilson will discuss your priorities, examine your breasts and explain what he believes can realistically be achieved.
